Helpful Questions

In order to more fully explore social health and to begin exploring spiritual health, it is important to gain your patient’s/client’s permission.
This could be done by asking a question such as:

Would you mind if I asked you some more personal questions so that I can better understand your situation and what really matters to you? Is that OK?
or
I would like to ask some more personal questions that could help me understand more about who you are, what matters most to you, and how you can be given the best care. Is that OK?


Questions that explore social health

What do you find satisfying or fulfilling about your work?
Could you tell me a bit about what you have done previously and your education?
Could you tell me a bit about your background and what it was it like growing up?
Could you tell me a bit about where you live and who you live with?
Do you have some relationships that are causing you stress or concern?
Do you have any concerns about your financial situation?
What do you enjoy doing outside of work? Are you part of any groups or clubs?


Questions that explore spiritual health

How are you feeling about your life at the moment?
What things in life really matter to you?
Is there something that you really want to achieve in life?
Who are the people that you feel most close to, that give you a sense of belonging?
What kind of things have you looked to for support when you’ve face hard situations in your life before?
What is it in your life that gives you the strength to keep going?
Is there anything that makes you feel hopeful, either for yourself or for others?
Are you the kind of person who has a faith or belief that helps you make sense of the world?
Are you the kind of person who finds prayer helpful?
Do your thoughts about what happens after death influence the way you think about your life?
Would you like to talk further with somebody about what we have discussed?
